The Jay clinic was moved to its present site in the former community hospital facility in 1989. It has a 28,345 sq. ft. floor space. After tribal take over of the existing facility a series of upgrades have resulted in a state of the art outpatient clinic. During FY 2008 there were 10,798 active patients who created 43,819 outpatient visits.

Scope of Service: The clinic provides Medical, Dental, Lab, Radiology, Public Health, WIC, Nutrition, Contract Care, Pharmacy, Behavioral Health, Optometry, Community Health Service and Mammography.

Hours of Operation: Monday through Friday from 7:00 a.m. until 5:30 p.m.
