Central Intake/Referral Form

Ages 14 Years thru 21 Years
The Summer Youth Employment Program is for youth age 14 through 21, residing within the 14 county jurisdictional areas.
The Central Intake/Referral Form is the first step in the application process for applying for the 2008 Summer Youth Employment Program. Please be sure to print and only fill the non-shaded areas of the application. If you are unsure of a question, we will be glad to assist you. There are additional forms that will need to be filled out at out office.
In addition to the two-page Central Intake/Referral Form, you will need to bring the following documents:
- Work Permit (if under 16 years of age)
- Verification of family income for the past six months
- Tribal Membership Card and/or CDIB
- Social Security Card
- School Verification (School ID, Current Report Card, etc.)
Applications will be taken:
March 17, 2008-May 16, 2008
Monday-Friday
8:00 a.m.-5:00 p.m.
Basic Eligibility Criteria:
- Reside within the Cherokee Nation 14 county service area.
- Meet income eligibility criteria.
- Be between the ages of fourteen (14) and twenty-one (21) years inclusive.
- Applicants ages 16-21 will be given priority for employment placement.
- Applicants ages 14 & 15 will be given priority to participate in Educational Enhancement.
- (Applicant should contact an SYEP Specialist for details)
- Be a member of a federally recognized tribe.
- Males will be required to register with the Selective Service System if 18 or older at the time of application.
*Applicants 17 years of age and younger must be accompanied by a parent or guardian during the application process.
Appointments can be scheduled to complete the application, if needed.
