·        Proof of income for all household members over age 18.

(If you are unemployed, one letter from another agency stating you are receiving assistance.)

·        CDIB card for EACH CHILD

(If you have recently applied, bring proof of application.)

·        Proof of address.  You must live with in the 14 counties of Cherokee Nation
